New Patient Registration

registration1The doctors welcome new patients who live within our practice area. 

For details of the Practice Area please see the map below. You can only be a registered patient with one GP surgery so if you register with us you will be de-registered with your current GP surgery and your previous medical notes will be sent to us.

Please call in at the surgery to complete the appropriate forms with a receptionist. 


Practice Boundary


Please see the Practice Boundary Map (Zone 1) to determine whether you live within the boundary and whether you can register with Cadwgan Surgery. For existing patients already registered with the practice who live in Zone 1 and moving out of the Practice Boundary you will need to register with a GP surgery elsewhere unless your new address is within Zone 2. Please telephone the Practice on 01492 515787 and ask to speak to Mrs Christine Jones if you require clarification about Zone 2 or any other matter to do with the boundary.
